Reservation Shop Order Form

Consent Form

In order to audio tape the test calls each participant needs to sign a consent form, providing the approval to audio tape over the phone.  Click here to download the Consent Form.  Please have each participant sign the form and forward it to our offices below.

Plug-in for Windows Media Player

We will forward you the audio of your test calls via email.  In order for you to play the audio on your computer, you will need to do a one-time download of this plug-in so your Windows Media Player will play the files.  Click on this Plug-in Link and choose Open.  The plug-in takes moments to install.  You can then begin listening to your audios!

Hotel Profile Form

If at any point you would also like to be set up on the Sales Department shopping program, we request that you fill out a Hotel Profile Form.
